If you are new to this area or would like to change your doctor, you may apply to join our surgery.
To join our practice, please complete the following documents. You may do by collecting the forms at the surgery. When you have completed the forms, present them to the surgery along with all required documentation (see right) at your first appointment.

• A medical questionnaire which will give us some basic information about you before your records arrive at the Practice.
• An application form. Please complete all appropriate sections and sign the form. On the back of the new forms there is a section asking for your wishes for organ donation; the information will go on a central computer database.
• A consent form for our appointment text messaging service.
